Financial Policy

PAYMENT IS REQUIRED AT THE TIME OF SERVICE UNLESS PREVIOUS ARRANGEMENTS HAVE BEEN MADE
In an effort to keep medical fees down while maintaining the highest level of professional care possible, we have established the following as our financial policies:

Payment is due at the time of service. Payment at the time of services saves us time, and expense in billing and carrying costs.

A $10.00 billing fee is charged to your account when payment is not made at the time of service.

Financial arrangements are available upon special request. Please contact the Billing Department prior to your scheduled office visit to make these arrangements.

The physician/patient relationship is a very special relationship that involves moral, ethical, and, more than ever, medicolegal implications. Part of the agreement of being a patient is payment of your responsible charges or co-payment at the time your services are rendered. We want to work with you and understand that some circumstances are extenuating. All that we ask is open communication and effort as you would expect from us.

Statements are sent out on a monthly basis. If, after 70 days, no payments have been received and no extended payment plans have been made and/or communication with our Billing Department, necessary collection proceedings will begin.

REFERRALS
Many of the HMO/PPO insurances we accept require a referral form be filled out by our physicians in order for you to:

• Have lab test;
• Have X-rays at another physician's office;
• See a specialist;
• Go to the Emergency Room;
• Go to the hospital;
• And others as the policy requires.

You must know how your HMO/PPO insurance works ( if you want them to cover your services). No matter how hard we try, we are not always able to remember each insurance policy. Please know which hospitalas your insurance participates with. If we work together on this, we will be able to prevent any insurance payment problems.

WARNING TO ALL MANAGED CARE PATIENTS:
Managed Care Patients may be charged for nonessential and non-authorized services. Our insurance contracts do not allow us to write referrals after the fact. Please get prior approval from us.
